Electrotherapy Instrument Purchasing Policy:
Doctor's Prescription Requirements:

With NO exception, ALL individuals purchasing Electrotherapy Instruments must Fax a Copy of Their Doctor's Prescription before ordering. Orders will not be shipped without the proper documentation.

Thirty (30) Day Return Policy:
Damaged or defective products:

All of your purchases at Chattanooga Medical Supply are covered for product defects for a full 30 days after receipt by you. In the unlikely event that you should receive defective merchandise, please contact our Customer Service Department immediately to determine the best method of product replacement or repair.

The 30 Days Described Above Refers to Chattanooga Medical's sales and does not apply to the manufacturers warranty, Most units are covered by a 1 year MANUFACTURER'S warranty and some by a 2 year Manufacturer's warranty, if something happens to your unit after 30 days, it will need to be handled by the manufacturer.

Beyond 30 days, if you have a one year warranty....you should contact the manufacturer for repair or replacement. This manufacturer contact information will usually be found on the box or in your warranty/instruction manual. If help is needed please don't hesitate to call us.

All products must be returned in the original condition including packaging, documentation, warranty cards, manuals and accessories. If we receive the product with damages, scratches or missing parts, In other words, in non resalable condition, we reserve the right to withhold our expenses to fix the damages from the customer's reimbursement.


 Refused by the recipient orders are considered unauthorized returns and therefore subject of the same fee and administrative charge. Customer will be responsible for the return shipping cost as well. If the customer returns the package due to a wrong street address or not matching recipient's name as provided by you in your order, The customer will be responsible for the shipping cost in both directions. If a reship is requested, the customer will also be responsible for the second shipping cost as well.



RETURN AUTHORIZATION

A Return Authorization Number (RA#) must be obtained from Chattanooga Medical Supply,Inc. before any items are returned in order for items to be accepted. All returns are subject to a 15% restocking fee. Shipping fees are non refundable.

RETURN EXCEPTIONS

Unfortunately, there are some products for which we are unable to accept returns or refunds, E.g., special order or production items, rush order items, Etc. If you are inquiring about any of the said items NOT displayed on the list, please contact us at our office number. 1-(423)-870-9030

For hygiene reasons, the U.S. Federal government forbids the resale of certain products considered to have potential for disease transmission.


These products include:

  • Foot Products - E.g. Superfeet Orthotics, Gel Insoles, Viscoped, Viscoheel
  • Pillows & Sleep Aids - E.g. Neck Support Pillows, Foam or Ortho Bed Wedges
  • Mattresses & Overlays - APM's, Overlays and Pads (vinyl & foam)
  •  Ultrasound Units, Electrical Stimulators, Electrodes of any sort that have been opened.

 

 IF YOU HAVE ANY discrepancies IN YOUR ORDER YOU HAVE FIVE (5) BUSINESS DAYS TO NOTIFY CHATTANOOGA MEDICAL SUPPLY, INC. OF SAID DISCREPANCIES OR YOU WILL BE SUBJECT TO FEES.
