REPAIRS & SERVICE

SCROLL DOWN FOR SHIPPING INFORMATION

Please note that repairs can take up to 2-3 weeks. Sometimes units can take months to fix, depending on if parts are in stock. Sending a unit in is a minimum of $82.50 (plus shipping charges), which is waived if the unit is chosen to be repaired with our facility.

HOW TO SHIP

Email us at Office@chattmed.com or phone us at 423-870-9030 to receive a prepaid shipping label and Equipment Service Repair Form. You may also download the form below, but you still must contact us to receive the prepaid shipping label.

Download and complete this form.
If form opens instead of download, to save right click the link and chose “download link as”.

Please ship this unit using the prepaid shipping label and place the completed Equipment Service Repair Form inside the box.

Please follow the instructions below:

  • Find a box that fits tightly around your unit.
  • Pack the unit extremely well to prevent damage during shipping. The unit should not be able to move inside the box.
  • Fill out the attached repair form and place it inside the box before sealing it.
  • Print the attached shipping label and tape it securely to the outside of the box.
  • Drop the package off at UPS. If you have a daily UPS pickup, they can take it as well.

Once we receive the unit, we will inspect it. If it only needs preventative maintenance or an intensity knob, the repair cost should not be much.

We will send you an estimate once the unit has been evaluated. If you approve the quote, we will generate an invoice for payment.

Once payment is processed by credit card, the unit will usually ship back out the same day.

If you choose not to proceed with the repair, we will put all of the old parts back on the unit and ship it back to you.

All units are calibrated by default, per FDA requirements.

IMPORTANT NOTES
Please make sure unit is packed extremely well to avoid damage in shipping. We suggest having the company you’re shipping with pack your equipment.

If you are sending in a unit, please include the soundheads* and lead wires if applicable. These are pertinent to diagnose and fix your unit. If the board is bad your equipment needs to be checked to make sure there aren’t faulty parts causing board errors, etc.

*DO NOT SEND IN ONLY SOUNDHEADS
Older soundheads that are no longer available for purchase can generally be fixed as long and the handle in intact, but often the cost of repairs is as much as a brand new unit.
